SUBJECT: COMBINED INSPECTION NOS. 50-277/93-08; 50-278/93-08

This letter transmits the findings of the special safety inspection conducted by

Messrs. A. L. Burritt and S. L. Hansell of this office from March 22 through

March 26,1993, at the Peach Bottom Atomic Power Station, Delta, Pennsylvania. At the

conclusion of the inspection, Mr. Hansell held discussions of our findings with members of

your staff.

The inspection was in the area of plant operations. The primary objective was to ensure that

the facility's ope. rations department conducted activities in a safe manner and in accordance

with both regulaory requirements and licensee approved procedures. A secondary objective

was to assess the interface and support between operations and the other departments as these

interactions occurred. Within the area, the inspection consisted of interviews with personnel,

observations by the inspector, and examinations of selected procedures and records.

In summary, operations were conducted safely, in accordance with regulatory requirements

and, in most instances, in accordance with licensee approved procedures. A strength was

noted in that the Alarm Response Cards contained detailed information and were routinely

used by the operations personnel.

There were two items of concern. The first was the operating shift's decision which resulted

in a reactor protection system bypass for the main turbine without pursuing a temporary

procedure change to accomplish the task. The second was the plant decision process to use

the Troubleshooting, Minor Rework, and Test Support (TMT) procedure instead of the

Clearance and Tagging procedure in one instance which could result in reduced personnel

safety. The facility has committed to conduct a formal review of both concerns. The details

for these two items are contained in report sections 3.6 and 3.7 respectively.

Within the scope of this inspection, no unresolved items or violations were identified. We

appreciate your cooperation.
