Dear Mr. Hall,
In accordance with Part III Section 12.C., this letter is serving as confirmation of a telephone report of an oil spill made to the State Emergency Response Commission on 4/8/93.
l 1.
The limitation which has been exceeded: The general effluent limitation requiring that the effluent shall be free of substances of an " oily, greasy, or surface active nature... in amounts that vill form a noticeable accumulation of scum, foam, or sheen.
Oil was discharged to a small, nameless stream on the east side of the plant site, creating a visible layer on the surface.

The extent of the exceedance: Approximately 35 gallons of oil vere discharged through plant site storm drains to the stream. The oil was contained in an impoundment area in the steam by.a skimmer vall. Samsel Services of Cleveland was contracted to perform clean up activities. The analytical result of a sample collected from downstream of the skimmer indicated that the oil was adequately contained behind the skimmer (the result vas below the lover limit of detection, 5.0 ppm).

The cause of the exceedance: Oil entered an on site veir structure as it l
vas being drained of water. The water was being pumped to the storm drain l
system. The oil was not noticed in the veir structure until some of it j
had been pumped to the storm drain.

The period of the exceedance: Approximately 0100 to 0150 on 4/8/93.

If uncorrected, the anticipated time the exceedance is expected to continue: The pumping activity was stopped at 0150 on 4/8/93.
Clean up activities in the stream vere completed by Samsel Services on 4/9/93.

Steps being taken to reduce, eliminate and/or prevent recurrence of the exceedance: A detailed engineering analysis is being conducted to determine the ultimate source of the oil. A Human Performance Enhancement System evaluation is also being conducted. The results of both evaluations vill be used to prevent recurrence.
