REACTIVE TEAM INSPECTION AT THE UNIVERSITY OF MICHIGAN FORD

NUCLEAR REACTOR

This refers to the reactive team inspection conducted by Mr. C. Cox and

j

Mr. T. Reidinger of this office and Mr._ T. Michaels and Mr. J. Arildsen of

i

the Office of Nuclear Reactor Regulation on March 25-26, 1993. The inspection

j

included a review of the overpower event on March 24, 1993 at the University

l

of Michigan Ford Nuclear Reactor facility. At the conclusion of the

!

inspection, the findings were discussed with those members of your staff.

j

identified in the enclosed report.

!

Areas examined during the inspection are identified in the report. Within

!

these areas, the inspection consisted of an examination of the procedures and

i

representative records regarding the March 24, 1993 event, interviews with

!

l

operations personnel, a walk-through of the event, and a review of the human

!

l

factors implications from the event.

!

Based on the results of this inspection, three apparent violations were

!

identified and are being considered for escalated enforcement. action in

i

accordance with the " General Statement of Policy and Procedure for NRC

~

Enforcement Actions" (Enforcement Policy),10 CFR Part 2, Appendix C.

These

i

apparent violations involved exceeding the license limit for thermal power and

i

two cases of a failure to follow procedures. Accordingly, no Notice of

j

Violation is presently being issued for these inspection findings.

addition, please be advised that the number and characterization of apparent

!

l

violations described in the enclosed inspection report may change as a result

j

'

of further NRC review.

j

An enforcement conference to. discuss these apparent violations has been

j

scheduled for April 26, 1993. This enforcement conference will be open to

j

public cbservation in accordance with the Commission's trial program as -

!

discussed in the enclosed Federal Register notice (Enclosure 2). The purposes

of this conference are to discuss _ the apparent violations, their causes and

i

safety significance; to provide you the opportunity to point out any errors in

l

our inspection report; and to provide an opportunity for you to present your

proposed corrective actions.

In addition, this is an opportunity for you to

i

i

provide any information concerning your perspective on (1) the severity of the
