As required by SQN Technical Specification (TS) Surveillance Requirement (SR) 4.4.5.5.b, enclosed are the results of the S/G tube ISI completed
.i for Unit.2 during the Unit 2 Cycle 5 refueling outage. The subject ISI i
was completed on April 19, 1992.
l The initial inspection samples in all four S/Gs were classified as Category C-2 (categories are defined in SR 4.4.5.2).
In S/Gs 1 and 4, the first expansion sample was classified as Category C-2.
In S/Gs 2 and
+
3, the first expansion sample was classified as Category C-2, while the second expansion sample (all remaining tubes) was classified as
'l Category C-1.
Table 6 in the enclosed report summarizes service-induced vall-lors indications, sample classification, and S/G inspection scope l
for the SQN Unit 2 Cycle 5 ISI.
TVA performed eddy current examinations such that the following total i
percentages of tubes were examined with bobbin coil probes full length, rotating pancake coil (RPC) at the hot-leg top-of-tubesheet (HTS), and RPC at the first hot-leg support plate (H01):

I As a result of these examinations, five tubes were plugged In S/G 1, nineteen tubes were plugged in S/G 2, twelve tubes in S/G 3, and two' tubes in S/G 4.
