Gentlemen:

SUBJECT: NRC INSPECTION REPORT NO. 50-327/93-10 AND 50-328/93-10

This refers to the special augmerted inspection team inspection led by

Jereme J. Blake of this office on March 3-11, 1993.

The inspection included a

review of the events associated with the electrical and secondary plant

transient which occurred at your Sequoyah Nuclear Plant on March 1,1993. At

the conclusion of the inspection, the findings were discussed with those

members of your staff identified in the report.

'

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinati6ns of procedures

.

and representative records, interviews with personnel, and otifervation of

l

activities in progress.

The findings of this inspection 'will be reviewed to

determine compliance with NRC regulatory requirements.

You will be notified

by separate correspondence if violations are identified.

The inspection revealed insufficient maintenance and surveillance of balance

{

of plant piping that resulted in significant degradation of this piping which

was not identified until the rupture of a ten inch extraction steam pipe ~on

March 1, 1993. The Sequoyah staff missed several opportunities to identify

the degraded piping, and the CHECMATE program which was intended to predict

piping degradation was not effective because of implementation deficiencies.

'

The root cause of these problems was a failure of management to clearly assign

responsibilities and provide appropriate oversight and direction for monitor-

ing and maintaining balance of plant piping.

Following the pipe rupture on March 1, 1993, you shut down both units and

initiated comprehensive corrective actions.

The NRC issued a confirmatory

'

action letter dated March 4,1993, to confirm actions to be taken prior to

restart of either unit. TVA will meet with NRC management in Washington, D.C.

on April 12, 1993, to discuss the status-of these corrective actions.
