Reply to Notice of Violation dated March 10,1993 Violation A Section 2.5, " Instrument and Equipment," Chapter 2 " Radiological Protection Standards," of the Safety Manual EMF-30, states in part:

"All radiation detection and dose rate instruments shall be inspected and calibrated per R gulatory Guide 8.24 at least semiannually."

Contrary to the above, all surface barrier detectors (5) in service at the time of the inspection had not been calibrated at least semiannually. The detectors, used by plant radiological safety for counJng routine and special air samples, had been in service since August 22, 1985.

i This is a severity level IV violation (Supplement IV).

SNP Repiv Reason for Violation - The violation was the result of an assumption that the daily source checks, in conjunction with chi-square testing of those detectors which fail the daily checks, constituted an acceptable calibration procedure.

Immediate Corrective Steps - An energy spectrum calibration was performed on February 16, 1992 for each of the surface barrier detectors.

Corrective Steps to Avoid Further Violations - Three procedures have been written regarding the calibration of the Tennelec surface barriers. Two Maintenance Instrument procedures, Calibration Procedures No.155 Rev. 2, effective March 9,1993 and No.156 Rev.1 effective February 22,1993 address energy calibration for the UO Building detectors and SF Building 2

detectors, respectively. Health Physics and Radiological Safety Procedure EMF-1507,2.2 entitled " Operation and Quality Control For Tennelec Surface Barrier Counters", effective May 1,1993, addresses Chi-square statistics, efficiency, and control charting.
