Closure of 10 CFR 21 Report l
l Consolidated Power Supply (CPS) reported a deficiency by telephone on October 20, 1992, and followed with the written report dated November 19, 1992.
The deficiency was noted as the inability to trace a 24" SA182 F316L, 150# RF blind flange, heat #460066, to the starting piece of material as required for compliance to ASME Code upgrade requirements. The material was manufactured by Texas Metal l
Works, Houston, TX.
The flange was supplied by CPS to Connex Pipe Systems, who in turn supplied the flange to Public Service Electric
& Gas.
After completing an internal investigation relative to the initial concern, CPS notified the applicable customers that were determined to be affected.
All such customers were notified in writing no later than November 19, 1992.
Several conversations have taken place in attempting to determine the status of the indeterminate materials. Out of the five customers originally notified, only two j
have responded in a manner to allow disposition of the material supplied and closure relative to their facilities.
These two are Connex Pipe Systems, who had three orders affected, and TU Electric.
The other three licensees have apparently not completed their evaluations at this time, and have provided no written l
response to CPS.
These three customers have conveyed by telephone that in process activities had been initiated.
i The ASME performed an audit at CPS on March 29, 30 and 31, 1993, which primarily focused on CPS upgrading activities.
The activities relative to the Texas Metal Works concern that have been i
conducted by CPS as of March 29, 1993 were evaluated by the audit team.
Due to the lack of response by end users combined with the fact that CPS had not received back several pieces of the sNE indeterminate materials, it was agreed during the audit that CPS f$$
would revise all original certification documents supplied with the material.
This was performed by CPS with certified mail being
,g utilized to send the correspondence, including cover letters, to all customers noted as affected by the upgrading deficiency.

Power Supply Letter to U.S. NRC Dated 4/2/93 Page 2 of 2 Code, including removal of the CPS ASME Quality System Certificate and expiration date.
The certification now reflects that the material was provided as commercial.
Please find enclosed a copy of the correspondence provided to the applicable customers, which includes a copy of the revised CPS certification documents.
- Also, find enclosed a copy of the CPS letter to ASME identifying what actions were taken as a result of the survey.
Once CPS receives acknowledgment from the certified mail system that all correspondence has been received, CPS will forward copics of such to ASME requesting closure of the finding.
Based on the above, CPS is closing the 10 CFR 21 report initiated on October 20, 1992.
We regret that all material has not been accounted for, however, four months should have been ample time for licensees to respond to our original request for assistance.
CPS management feels everything possible has been performed in an effort to address the original deficiency.

Dear Mr. Karian:
i Upon review of the referenced document, it is apparent that Consolidated Power Supply (CPS) has not utilized sufficient measures in order to address the open concern.
As a result, the affected customers that have not provided documentation to CPS to allow closure of the deficiency have been notified in writing of the additional measures now being implemented.
CPS has revised the certification documents originally provided i
with the indeterminate material deleting all references to qualification to the ASME Code.
This includes the deletion of the i
CPS Quality System Certificate number (515) and expiration date.
Please find enclosed copies of the revised certification, which are attached to the correspondence issued to each applicable customer.
The correspondence is being mailed today as certified mail, requiring acknowledgment by the recipient.
Once verification has been received by CPS that the letters were received by each customer, you will be notified immediately that all measures possible have been implemented by CPS and will request closure of this concern.
In addition, ASME audit now in progress combined with the above will hopefully reflect sufficient information to substantiate the fact that CPS does recognize the ASME Code upgrade requirements in their entirety.
In addition to the follow up activities for the Texas Metal Works concern, other areas are being addressed by the ASME Audit Team.

Follow Up on the 10 CFR 21 Report for Connex Pipe Systems Purchase Order E4210-2
Dear Mr. Welch:
As we have discussed, the 24" blind flange supplied by Consolidated Power Supply (CPS) remains an open issue.
This is due to the indeterminate status of the one piece of material.
I have attempted to contact Mr. Dave Perkins, Public Service Electric &
Gas, relative to whether or not the flange has been removed from service, and if it was removed where the material is physically located.

As there has been no conveyance from PSE&G to date regarding the definite status of the material, CPS is implementing the actions necessary to initiate closure of the open deficiency.
Please find enclosed with this correspondence a revised CPS certification document for the subject material.
The revision deleted the references to the ASME Code requirements, including the reference to QSC# 515.
The revised certification shall be utilized as a replacement for the original certification that was provided for i
the subject material.
By issuance and verification of your receipt of this letter, the 10 CFR 21 report relative to Connex Pipe Systems will be considered closed.
It is required that you forward the enclosed certification document to Mr. Perkins as PSE&G is the end user of the material.
Closure is also based on the return of all deficient material identified on Connex Pipe System Purchase Orders E4268-1 and E4268-2.
