NRC INSPECTION REPORT NO. 50-498/93-07; 50-199/93-07

During February 4-24, 1993, an NRC Augmented Inspection Team (AIT) conducted

an onsite inspection at the South Texas Project Electric Generating

Station (STPEGS).

The AIT reviewed the circumstances following events at

STPEGS that occurred during the period of December 27, 1992, through

Febraary 3,1993, involving overspeed tripping of the turbine driven auxiliary

feedwater pumps.

The enclosed copy of the All report describes the tasks assigned to the AIT,

the areas examined during this inspection, and our conclusions and findings.

During an interim exit meeting conducted on February ll, 1993, you committed

to performing certain testing, procedure revisions, training, and

modifications to ensure that the turbine driven auxiliary feedwater pumps do

not trip on overspeed when required for operation. Details of these

commitments are provided in Section 4 of this report.

After the inspection, our findings and your committments were discussed with

Houston Lighting & Power personnel and members of the press and public during

a public exit meeting held on February 24, 1993.

An additional public meeting

will be conducted at your convenience to brief the NRC, as committed to in the

Confirmatory Action Letter dated February 5, 1993.

This inspection identified weaknesses with your maintenance program and

corrective action processes.

We note that, had your maintenance program and

corrective action processes been effective, these overspeed events would

probably not have occurred.

A followup inspection was conducted during

March 8-12, 1993, to address this area and this inspection report (NRC

Inspection Report 50-489/93-05; 50-499/93-05) will be issued in the near

future.

In accordance with 10 CFR Part 2.790(a), a copy of this letter and the

enclosure will be placed in the NRC Public Document Room.
