SUBJECT: REACTIVE SAFETY INSPECTION OF THE OHIO STATE UNIVERSITY RESEARCH
REACTOR
This refers to the reactive safety inspection conducted by Mr. C. Cox of.this

office on March 9, 1993. The inspection included a review of activities
authorized for the Ohio State University Research Reactor. At the conclusion
of the inspection, the findings were discussed with those members of your
staff identified in the enclosed inspection report.

Areas examined during the inspection are identified in the enclosed report.
.
Within these areas, the inspection consisted of reviewing records of the shim
safety rod event, examination of the equipment involved in the event, and
!
observation of the corrective action activities in progress.

No violations of NRC requirements were identified during the course of this
j

inspection.

The NRC was concerned with the recent failure of a shim safety rod.to insert
when a manual scram signal was initiated. We acknowledge your cooperation by

prohibiting operations until the cause of the event was determined and-
I'
assessed and all appropriate testing had bean performed and reviewed with
d
representatives of the NRC Region III staff. We appreciate your cooperation
in this matter and encourage you to continue this open channel of
f
communication between your staff and mine.
