NRC INSPECTION REPORT N0. 70-1257/93-02 This refers to the special inspection conducted by the Nuclear Regulatory Commission's (NRC) Augmented Inspection Team (AIT) at your Richland facility on February 9-12, 1993. The inspection was in response to an event on February 7,1993, involving the release of a large quantity of low enriched uranium from its normal process system. At the conclusion of the inspection on February 12, 1993, the AIT discussed its findings with you and members of your staff at an open meeting.

The AIT's charter, the areas examined during this inspection, and the AIT's findings and conclusions are described in the enclosed inspection report. The inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the AIT.

The AIT found the cause of this event to be rooted in poor procedural compliance and communications. The AIT also identified concerns in your work control practices and in criticality safety relative to the potential for neutron moderator intrusion into your controlled process areas. The overall conclusions of the AIT are found in Section 10.0 of the enclosed report. A Confirmatory Action Letter was issued on February 12, 1993, regarding the moderation control issue. Other than the moderation control issue, we note that your investigation arrived at similar conclusions as the AIT.

This report does not address enforcement issues. Any enforcement issues evolving from this event will be handled separately.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosures will be placed in the NRC's Public Document Room.
