SUBJECT: NRC INSPECTION REPORT. 50-072/93-01; 50-407/93-01
This refers to the inspection conducted by Dr. J. B. Nicholas and
' Dr. D. B. Spitzberg of. this office during the period February 8-12,:1993. The
.
inspection included a review of activities authorized for' the University of
Utah's AGN-201 and TRIGA reactors. At the conclusion of the inspection, the -
findings were discussed with you and those members of the university staff
identified in the enclosed report.-
Areas examined during the inspection are identified in the.. report. These
areas included the reactor operations, nuclear material control'and
accountability, radiation protection, emergency l preparedness, and physical
.
security programs. Within these areas,- the inspection consisted of selective
examination of procedures and ' representative records, interviews with .

. personnel, and observation.of activities in progress. The results of this :

inspection are documented in the enclosed report.,
During this inspection, it was identified thatLone of'your licensed ' senior'
q
reactor. operators had failed to complete his biennial operator requalification
requirements' by not taking the 1992 annual written operator requalification
-
examination, and this was _ in violation of Section V.A of your NRC' approved

reactor operator.requalification training program. .It wasialso identified
'
that another' one of your licensed senior . reactor operators had-also' failed to
complete his biennial operator requalification requirements by not~ taking his
biennial medical examination, and this was in violation'of'Section VII. of your
C.
NRC approved reactor operator requalification training program and..
10 CFR 55.53(i) requirements. These violations are'not beingicited because-
the cr.iteria in paragraph VII.B.1 of Appendix C to 10 CFR Part. 2 of the NRC's:
" Rules of Practice," were satisfied. These violations were isolated

occurrences and the licensee's staff took prompt and effective corrective

. actions to correct the problems.-
It was also identified that'your annual reactor operating report for the
reporting. period July 1,1990, through June 30, 1991, was submitted after the
deadline.of 60 days following .the 30th of June of each year,:and this was a
- '
licensee-identified violation of Technical Specification 6.10(5).. This
~
violation is not being cited because the criteria in paragraph VII.B'.2 of
Appendix C to 10 CFR Part 2 of the NRC's " Rules of Practice,"'were satisfied.
In the area of emergency preparedness, two weaknesses were identified. ione
weakness is discussed in Section.13.1.3 of the enclosed report and involved
the failure to demonstrate the capability to promptly classify. emergency.
events according to the actual or projected radiological consequences.-
specified in your emergency action level scheme. -The second weakness is
discussed in Section 13.1.4 of the report and involved the absence of an
agreement between the licensee and ambulance providers. You are requested to
provide this office, within 30 days after receipt of this letter, with your.
analysis of the weaknesses, a description of your corrective measures, and-
your schedule for completing these actions.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," .a copy of
this letter and its enclosures will be placed in the NRC Public Document Room.
However, in accordance with'10 CFR Part 2.790(d), the documentation enclosed
concerning your Physical Security Plan (Attachment 2 to the Appendix) is
exempt from disclosure. Therefore, this Attachment will not be placed in the
Public Document Room.
The responses directed by this letter are not. subject to the clearance
procedures of the Office of Management and Budget, as. required by the:
Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
