SUBJECT:
TERMINATION OF NRC LICENSE No. 50-29009-01
This refers to the U. S. Nuclear Regulatory Commission-(NRC)
Region V letter dated December 22, 1992, which terminated License
No. 50-29009-01 in accordance with Amendment No. 01.
Since
Amendment No. 1 terminated your license, no response to the
Demand for Information; NRC Inspection Report No. 030-31493/92-01
dated November 10, 1992, was required.
The apparent. violations
and associated willfulness would normally'have warranted
escalated action, such as the issuance of a civil penalty or an
order _to modify, suspend, or revoke,your license.
Such action is
not being taken because you requested that your license be
terminated.
Should you, at a future time, apply for a new NRC
license, you will be required to respond to the apparent-
violations and the Demand prior to any licensing action by the
NRC.
In accordance with 10 CFR 2.790 of the NRC's " Rules of-Practice,"
a copy of this letter and its enclosure will be placed in the NRC
Public Document Room.
