NRC INSPECTION REPORT NO. 50-498/92-36; 50-499/92-36 (NOTICE OF

VIOLATION)

This refers to the inspection conducted by Mr. J. I. Tapit and the inspectors

!

identified in the attached report, of this office between December 6, 1992,

and January 16, 1993. The inspection ' included a review of activities

authorized for your South Texas Project, Units 1 and 2, facility. At the

,

conclusion of the inspection, the findings were discussed with those members

of your staff identified in the enclosed report.

,

Areas examined during the ic.spection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observations of

,

activities in progress.

Based on the results of this inspection, certain licensee activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice).

The first violation is of concern because it represents

five examples where the requirements of the Technical Specifications were not

adhered to.

The second violation relates to less than adequate preventive

maintenance on an important safety-related component, which caused a

subsequent valid failure of that component. The third and fourth violation

.

represent failures to satisfy the requirements of the American Society of

Mechanical Engineer's Code requirements.

)

During this inspection period, your staff identified that selected Technical

i

Specification surveillance procedures were less than adequate. Although this

constituted a violation of NRC requirements, the violations are not being

cited because the criteria in paragraph VII.B.2.of Appendix C to 10 CFR Part 2

I

of the NRC's " Rules of Practice," were satisfied. These violations were

licensee identified and your staff took prompt and effective actions to

,

correct the problem. One of the two violations was discovered during a

procedure review that was a corrective action for a previous licensee event

report. The other violation was found during an engineering review of

completed surveillance procedures.

The response directed by this letter and enclosed Notice are not subject to

the clearance procedures of the Office of Management and Budget, as required

by the Paperwork Redoction Act of 1980, PL 96-511.
