NRC Bulletin No. 90-01, Supplement 1: Loss of Fill-Oil in Transmitters Manufactured by Rosemount Gentiemen:
NRC Bulletin 90-01, Reference (b), informeu licensees that-reported failures of Rosemount Models 1153 and 1154 transmitters were caused by a gradual loss of fill-oil from the transmitters' sealed sensing module. In order to preclude further failures, the NRC requested licensees to promptly identify and take appropriate actions for Model 1153 Series B, Model 1153 Series D, and Model 1154 transmitters manufactured by Rosemount that may be leaking fill-oil. Reference (b) further requested licensees j
to provide a response that confirms the requested actions have been completed and that provides certain other specified information concerning transmitters suspected i
or confirmed of having a loss of fill-oil or from the high failure fraction lots.
By Reference (c), Maine Yankee stated "pending receipt of additional guidance providing a time at pressure basis for acceptance of our present transmitters, Maine Yankee intends to replace all Model 1153 Series B, Model 1153 Series D and Model 1154 transmitters manufactured by Rosemount prior to July 11,1989 installed in safety-class-systems with transmitters manufactured after July 11, 1989 or restored to post July 11, 1989 standards consistent with Rosemount guidance. We plan to complete the repair / replacement of those transmitters by the end of our next_ refueling outage."
By Reference (d), Maine Yankee provided information. relative to a modification in our plan to close out Reference (b) that we considered consistent with the NUMARC Rosemount database.
This modification would have accepted 32 transmitters because of low pressure or exceedence of the Rosemount psi-month criteria.

discussions with staff personnel during the early fall of 1991 indicated that our conclusions relative to the NUMARC database could not be supported at that time. As i
a result, action was initiated to repair these transmitters during the next outage consistent with Reference (c). This repair / replacement action was completed in April 1992.

Review plant records and identify any Rosemount Model 1153 Series B, Model 1153 l
Series D, and Model 1154 transmitters manufactured before July 11, 1989, that are used or may be used in the future in either safety-related systems or systems installed in accordance with 10 CFR 50.62 (the ATWS rule).

All mohl 1153 Series B,1153 Series D, and Model 1154 Rosemount transmitters I
manufactured prior to July 11, 1989 and installed at Maine Yankee in either safety-class systems or systems installed in accordance with the ATWS rule have been identified and replaced or repaired to post July 11, 1989 standards. Specifically, i
all replacement transmitters have serial numbers greater than 500000 and all repaired transmitters have sensor module numbers greater than 2182605. In addition, all spare i
Rosemount transmitters and sensor modules which may be used in the future meet this criteria.
A listing of all installed Model 1153/1154 transmitters subject to Reference (e) requirements is attached.

NRC Reauested Action Item No. 2:
Eveluate the enhanced surveillance menitoring program to ensure that the program provides measurement data with an accuracy range consistent with that needed for comparison with manufacturer drift data criteria for determining degradation caused by a loss of fill-oil.
Maine Yankee Response:
As all transmitters or transmitter sensing modules have been replaced, an enhanced monitoring program is not required.
Maine Yankee specifically chose to replace rather than become encumbered by a monitoring program. Thus, Items 1 and 2 of Requested Actions for Operating Reactors of Reference (e) have been completed.
