SUBJECT: NRC INSPECTION REPORT NO. 50-333/92-21

On December 2,3,4, and 23,1992, an inspection was performed by'Mr. M. Modes of the
Fitzpatrick piping radiographic program. The inspection identified several deficiencies which
were discussed with you at the December 4 and 23,1992 exit interviews at the conclusion of
,
the inspection.
Based on the results of the inspection, certain of your activities appeared to be in violation of L
NRC requirements, as specified in the enclosed Notice of Violation (Notice). The violations
are of concern because the quality of radiography helps to ensure the integrity of the ' afety
s
related piping systems. You are required to respond to this letter and should follow the -
instructions specified in the enclosed Notice when preparing your response. In your response -
!
you should document the specific actions taken and any additional actions you plan to prevent
recurrence. We have reviewed your initial corrective actions and concluded they were
adequate to support restart of the facility.
The responses directed by this letter and the enclosed Notice are not subject to the clearance
procedures of the Office of Management and Budget as required by the Paperwork Reduction
