Unit Nos.1 & 2; Docket Nos. 50-317 & 50-318 Annual Report of Failures and Challenges to PORVs and Safety Valves for 1

the Period of January 1.1992 throuch December 31,1997 i

l Gentlemen:

l 1

This report is submitted in accordance with Technical Specification 6.9.1.5.c.

The Technical Specification requires an annual report be submitted to the NRC detailing all challenges to, and failures of, the Power Operated Relief Valves (PORV) and Safety Valves.

l One Pressurizer Safety Valve failed its Lift Pressure Setpoint Surveillance during 1992. On August 15, during the performance of an ASME Boiler and Pressure Vessel Code (B&PV)

Section XI inspection,1-RV-200 (Unit 1) opened below the tolerance range of its required lift setting. Mechanical Maintenance Surveillance Test, M-2-1 " Pressurizer Safety Valves" requires the valve to lift within +/- 1Fo of its Technical Specification value. The valve, one of the two Unit 1 pressurizer safety valves, has a lift point o 2500 psia +/- 1Fo (2475 to 2525 psia). It lifted at r

2463 psia. Since 1-RV-200 failed its test, the ASME B&PV Code required the second pressurizer safety valve,1-RV-201, to be tested.1-RV-201 opened within its required tolerance range. The setpoint of 1-RV-200 was adjusted and the valve was tested satisfactorily.

Should you have any further questions regarding this matter, we will be pleased to discuss them with you.

Very truly yours, p.
