NRC INSPECTION REPORT NOS. 50-327/93-02 AND 50-328/93-0Z

This refers to the reactive team inspection conducted by the Nuclear

!

Regulatory Commission (NRC) at your Sequoyah Nuclear Plant facility during the -

1

period January 11-22, 1993. The inspection included a review of the events at

Sequoyah that led to the dual unit reactor trip on the evening of December 31,

l

1992. The inspection team also reviewed several recent Sequoyah single and

L

dual unit operational transients for the impact of shared systems and other

factors as a common cause. On February 5, 1993, at the exit meeting for the

inspection, the findings were discussed with those members of your staff

identified in the enclosed inspection report.

'

The enclosed copy of the inspection report identifies the areas examined by

the team. Within these areas, the inspection consisted of selective

i

examinations of procedures and representative records, interviews with

i

personnel, and observation of activities in progress.

,

!

Based on the results of the inspection, two apparent violations were

identified and are being considered for escalated enforcement action in

,

accordance with the " General Statement of Policy and Procedure for NRC

i

Enforcement Actions" (Enforcement Policy),-10 CFR Part 2, Appendix C, (57FR

j

5791, Febuary 18,1992). The apparent violations involved inadequate

!

procedures and failure to follow procedures, as well as inadequate corrective

r

actions, which resulted in the December 31, 1992, Unit 2 loss of reactor

,

coolant pump (RCP) seal cooling event which is a significant precursor to a

RCP seal loss of coolant accident (LOCA). Accordingly, no Notice of Violation

i

is presently being issued for these inspection findings.

that the number and characterization of ~ the apparent violations described in

the enclosed inspection report may change as a result of further NRC review.

An enforcement conference has been scheduled for March 3,1993, in the

Region II Office. The purposes of this conference are to discuss the apparent.

violations, their cause and safety significance; to provide you the

opportunity to point out any errors in our inspection report; to provide an

discuss any other information that will help us determine the appropriate

enforcement action in accordance with the Enforcement Policy.

You are

specifically requested to address the impact of limited control room staffing

'

as it related to this event.

You will be advised by separate correspondence

of the results of our deliberations on this matter. No response'regarding the

'

apparent violation is required at this time.

In addition, based on the results of the inspection, certain of your

.

!

activities appeared to violate NRC requirements as described in the enclosed

Notice of Violation (Notice). This violation involved failure to follow

!

procedures with multiple examples and indicate that, until recently, -your

sensitivity to the impact of secondary plant design, operation, material

,

condition and work control on plant transients was less than adequate. These

l

concerns include'the lack of proper management controls / oversight of sensitive

1

operations in your switchyard. The team noted the initiation of the Secondary

'

Plant Reliability study as well as the Sequoyah. Improvement Plan (SIP) now

.l

underway at the site. While these efforts have the potential to effect

performance improvement, it will require timely completion of the development

and continued Sequoyah management leadership and support to succeed.

In accordance with Section 2 790(a) of the NRC's " Rules of Practice," a copy

of this letter and its enclosures will be placed in the NRC's Public Document

Room.

The responses directed by this letter and its enclosures are not subject to

the clearance procedures of the Office of Management and Budget as required by

the Paperwork Reduction Act of 1980, PL 96-511.
