Modification No. 5 to Task Order No. 1 Under Contract No. NRC-33-91-219 The purpose of this modification is to:
(1) increase the level of effort which shall be performed in accordance with the enclosed Statement of Work, and (2) increase the contract ceiling amount by $14,403.96 from 5689,937.98 to 5704,341.94.
The effective date of this modification is October 1, 1992.
Accordingly, the task order is hereby modified as follows:
The second paragraph of the definitization letter is deleted in its entirety and the following paragraph is substituted in lieu thereof:
" Task Order No. 001 shall be in effect from July 20, 1992 through July 19, 1994 with a total cost ceiling of $704,341.94.
The amount of
$655,455.75 represents the estimated reimbursable costs and the amount of $48,886.13 represents the fixed fee."
Issuance of

a The following changes are to be incorporated in the Statement of Work of the f

Section C, Work Requirements i
The pager required for the I-Net personnel shall be provided by the contractor.
The following is added to Section C.2.8, Customer Support-o I

The contractor shall provide a "helpdesk" package for documenting, tracking and monitoring customer support calls for network l
support. As a minimum, the package shall provide documentation j

for (1) user name, (2) date and time of call received, (3) name of.
j file server, (4) problem type, (5) problem assigned to, and (6) resolution of problem.

The contractor shall provide on-call service for expandeu aours of o
coverage for network support as follows:
