at the Millstone Nuclear Power Facility, Unit 2. Tbc details are enclosed for your review and t

We request that the results of your review and disposition of these matters be submitted to f

Region I within 30 days of the date of receipt of this letter. We request that your response-contain no personal privacy, proprietary, or safeguards information so it can be released to i

the public and placed in the NRC Public Document Room. If necessary, such information j

shall be contained in a separate attachment which will be withheld from public disclosure.

The affidavit required by 10 CFR 2.790(b) must accompany your response if proprietary -

i information is included. Please refer to file number RI-91-A-0046 wher. providing your response.

The enclosure to this letter should be controlled and distribution limited to personne wnn a' "need to know" until your investigation of :he concern has been completed and reviewed by -

NRC Region 1. The enclosure to this letter is considered Exempt from Public Disclosure in accordance with Title 10, Code of Federal Regulations, Part 2.790(a). However, a copy of j

this letter excluding the enclosure will be placed in the NRC Public Document room.

The response requested by this letter and the accompanying enclosure are not subject to thc clearance procedures of the Office of Management and Btidget as required by the Paperwork ~

Issue 1; An Instrument and Controls technician (*) worked on RPS matrix testing and NI calibrations to support the hiillstone 2 stanup following a scram en February 16,1991. The surveillance procedure had undergone extensive changes recently and the technician was not I

adequately trained on the recent revisions to the procedure. Special assistance was required for the technician to understand and complete the procedure.

Please discuss the validity of the above assenion. Please discuss any corrective actions being taken to train technicians on revisions to procedures prior to implementation.

(*) The identity of the individual may be obtained from the Senior Resident inspector.

Issue 2; Hours in excess of overtime limitatier.s. were worked by Instrument and Controls technicians (*) in suppon of the hiillstone 2 stanup on February 17, 1991. One individual

(") was "on-call" for plant suppon activities and was in the dual role of being "on-call" as an

emergency responder. This technician worked a 10 hour1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br /> shift on hiillstone 3 un February 16 l

and remained on duty for 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> keeping the pager until 0730 on February 17. This indisidual was overworked and could not perform his duties in suppon of the unit stanup.

Please discuss the validity of the above assertions. Please discuss any actions taken to ensure overtime restrictions are not exceeded in cases such as that described above.
