e-m NRC-04-88-108 Modification No. 4 Page 2 This modification is issued to (1) extend the duration of the contract; (2) modify the Statement of Work (50W) to delete the requirement for a report under Subtask II a for the third ILRT; (3) deobligate $25,000 for the decreased S0W; (4) add a name under the " Key Personnel" Clause; and (5) accept the Contractor's March 15, 1990 written offer of consideration for these actions. Accordingly, the contract is hereby revised to read as follows:
1.
Subsection B.2 - Consideration and Obligation--Firm Fixed Price-is hereby decreased by $25,000.00 from $150,000.00 to $125,000.00 as follows:
"B.2 CONSIDERATION AND OBLIGATION--FIRM FIXED PRICE (MAR 1987)
The firm fixed price of this contract is $125,000.00.
Payment will be made upon completion / delivery and acceptance of all required products / services in accordance with Section F of this contract."
2.
Under subsection C.1 - Statement of Work, the Contractor's proposal dated September 15, 1988, DELETE in its entirety,the following:
"PART 2: Milestone 2 -- completion of the third of three (3)
ILRTs proposed in subtask II.a; submittal is report for this ILRT to NRC."
3.
Subsection F.6 - Ruration of Contract Period is hereby. revised to read as follows:
"F.6 DURATION OF CONTRACT PERIOD (MAR 1987)
ALTERNATE II (MAR 1987)
This contract shall commence on 9/30/88 and will expire on 8/31/90."
4.
Paragraph B. of subsection F.7 Deliverables aid Payments, is revised to read as follows:
"B.
Payments shall be made in accordance with the following:
(1) ILRT Performance *...............................$16,000.00
'(2) Report Prep *
...................................$ 9,000.00 (3) After receipt and acceptance of Accumulative Final Report referenced in Subtask IIc of.the proposal.
