APPENDIX A NOTICE OF VIOLATION New York Power Authority Docket No. 50-286 Indian Point Unit 3 Nuclear Power Plant License No. DPR-64 As a result of the inspection conducted on March 12 to March 16, 1990, and in accordance with the NRC Enforcement Policy, (10 CFR Part.2, Appendix C), thel following violation was identified.

The Indian Point 3 Nuclear Power Plant Guard Training and Qualification _ Plan,-

dated August 2,1982, Revis_ ion 3, Section 1.4. states, in part, that security force members will be tested at least once every 12 months to ensure they meet-the vision, hearing, disease and other physical requirements of 10 CFR-73',

Appendix 8.

Appendix B to 10 CFR 73, General Criteria for Security Personnel,Section I, B.b.(1), Vision (a), states that glaucoma shall be disqualifying, unless controlled by acceptable medical or surgical means.

Contrary to the above, on March 15, 1990, tne. inspector _ determined that' security personnel were not being tested for glaucoma.

This is a Severity Level V Violation (Supplement III)

,.I Pursuant to the provisions of 10 CFR 2.201, New York Power Authority is hereby j

required to submit to this office within thirty days of the date of the. letter which transmitted this Notice, a written _ statement or explanation in reply, including: (1) the corrective steps which have been taken and the results 4

achieved; (2) corrective steps which will be taken to avoid further violations; l

and (3) the date when full compliance will be echieved. Where good cause is shown, consideration will be given to extending this' response time.

All Safeguards Information (SGI), as defined in 10 CF6 73.21, contained in your submittal should be included in an enclosure so that the transmittal letter without'the SGI can be placed in the NRC's Public Document Room in accordance with 10 CFR 2.790(a).
