Dear Mr. Trowbridge:
This refers to Certificate of Compliance No. 9024 for the Model No. STL-201 packaging which has an expiration date of May 31, 1990.
To ensure continued use of this package, yoJ should submit an application (6
. copies) for renewal of the certificate at least 30 days prior to the expiration date. The sections on operating procedures, acceptance tests, and maintenance programs should be reviewed to assure they are complete and current.
If revisions are necessary, the revised sections should be submitted with the application for renewal.
Note that the certificate of compliance may be conditioned to specifically cite the operating procedures, acceptance testing and maintenance programs.
Unless an exemption is provided by 10 CFR Part 170.11, an application fee of
$150.00, as required by 10 CFR Part 170.31, should be included with the request for renewal. This notice of expiration should not be construed that such notice will be provided in the future. Also, please let us know if you do not intend to request renewal of the certificate.
