Gentlemen:
The documentation of all failures and challenges to pressurizer PORVs and safety valves for the. period of January 1,
1989 through December 31, 1989, follows. This.
report is required by Technical Specification 6.9.1.5.c.
On March 23, 1989, during' the performance of an ASME. Boiler and Pressure Vessel (B&PV) Code Section - XI inspection, one pressurizer safety valve ' _ opened below the tolerance range - of-its required lift-setting, Mechanical Maintenance Surveillance ' Test, M-2-2 " Pressurizer Safety Valves" requires the valves to lift N
within Li 1%Jof their Technical Specification value. ' RC-200, one of two Unit-2 pressurizer safety. valves, has a ' lift point of 2500 psia 1%. It lifted at 2412 psia.
Since RC-200 - failed - its test, the ASME B&PV Code requires the second pressurizer safety < valve, RC-201, to be tested, RC-201 opened within its required _ tolerance _ range.' The setpoints of RC-200 were-adjusted and the valve was tested satisfactorily. A records review concluded that no maintenance had been performed on RC-200 since it successfully passed an October 1985 surveillance test.
