APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket No. 50-423 Millstone Nuclear Power Station, Unit 3 License No. CPPR-113 As a result of the inspection conducted September 28-30 and October 1-23, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
A.
10 CFR 50, Appendix B, states in part, "XVI Corrective Action - Measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment, and nonconformances are p;omptly identified and corrected..."
The Northeast Utilities QA Program Topical Report Section 16.2.2 states that:
"The organization which is responsible for the condition adverse to quality, should ensure that the corrective action shall not only correct the immediate condition, but also prevent the adverse condition from re-curring."
The Stone & Webster Quality Standard Procedure QA-15.1, in section 7.7 states, "upon completion of the disposition action of the nonconformance and disposition report (N&D), field quality control shall perform inspec-tion / verification as required and indicate whether the completed action is acceptable or unacceptable followed by a signature and date in the "in-spection/ verification" block of the N&D Report".
Contrary to the above, on August 29, 1981, Stone and Webster field quality control verified that the completed action to disposition Nonconformance and Disposition report No. 0529 was acceptable, when in fact, it was not.
Eight previously identified weld defects associated with this N&D had not been corrected as required.
This is a Severity Level V Violation (Supplement II).
B.
10 CFR 50, Appendix B, states in part, "VI Document Control - Measures shall be established to control the issuance of documents, such as in-structions, procedures, and drawings, including changes thereto, which prescribe all activities affecting quality. These measures shall assure that documents, inicuding changes, are reveiwed for adequacy and approved for release by authorized personnel and are distributed to and used as l
the location where the prescribed activity is performed...."
The Northeast Utilities QA Program Topical Report, Section 5.2 states, in part, that:
"During construction the contractor programs are re-quired to clearly delineate the actions to be accomplished in the...
control of procedures, instructions and drawings and the methods for complying with 10 CFR 50, Appendix B".

The Stone & Webster Construction Methods Procedure No. 11.1-9.80, en-titled " Job Site Document Control" states in section 3.3 that, "the distribution of all level 1 documents shall be controlled by Document Control Section to assure only correct and up-to-date information is used for performing the work".
Contrary to the above, on October 9,1981, twelve drawings at drawing stations Nos. 40, 44 and 58 were not correct and up-to-data.
This is a Severity Level IV Violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.
The responses directed by this Notice are not subject to the clearance pro-cedures of the Office of Management and Budget as required by the Paperwork Reduction Action of 1980, PL 96-511.
