/., v Description Occurrence:
i On 11/18/81 during normal operation, operations personnel attempted to -
cycle reactor water cleanup system return isolation valve MOV-2740. An investigation was initiated after it was found that no flow changes occurred in response to tne valve being cycled. The investigation revealed that the valve stem clamp setscrew had become loose which allowed the clamp key to drop out. This allowed the stem to rotate whenever the motor operator was cycled and resulted in no valve disk movement. The isolation function of the valve was thus compromised. The key has been reinstalled and the stem ' clamp restored to its original configuration. M0V 2740 is a 4 inch anchor globe valve.
