Bechtel field engineering has been requested to generate a non-t conformance report against valves 2MO-1111, 2M0-1101B, and 2MO-1120B purchased from Westinghouse Electric on Purchase Order 7220-M-123A(Q) -AC, Items 4.2, 5.4, and 5.8, respectively. These three valv.s have environmentally unqualified terminal blocks (Buchanan 0824),
as reported in Item B of MCAR 46, Interin Report 4, dated August 28, 1981.

A Consumers Power Company /Bechtel/Limitorque meeting was held at Limitorque's Lynchburg office on September 17, 1981, to review and resolve environmental qualifications for terminal biceks and schedule insp(ction and replacement of unqualified components. Based on discussions at the meeting and a November 3, 1981, telephone conver-sation, the status for each terminal block type is as follows.
i a.
Buchanan 0222 This terminal block is correctly rated for 460 V use and is environmentally qualified for inside and outside containment area. No replacement will be necessary. Limitorque will furnish complete documentation by December 31, 1981.
b.
Buchanan 0524 This terminal block is correctly rated for 460 V use and is qualified for outside containment use by analysis.
It is expected to be qualified for in-containment usc by December 31, -
1981, by actual testing. No replacement is presently required.
Limitorque will furnish complete documentation by December 31, 1981.
c.
Euchanan 0724 Based on verbal discussions, this type was incorrectly identified in Limitorque's letter to Bechtel, dated June 25, 1981. The correct type is Buchanan 0824, in accordance with Limitorque's letter to Bechtel, dated July 31, 1981.

Buchanan 0824 This terminal block is correctly rated for 460 V use.
Because Limitorque does not intend to qualify this type, it will be replaced by Curtis Type L terminal block, rather than Mara-thon 300 Series terminal block because of the need for larger amperage rating.

Curtis Type L This terminal block type is correctly rated for 460 V use, and j
is analytically qualified for inside containment use. Actual testing is expected to be fully complete by December 31, 1981.
Limitorque will furnish documentation by December 31, 1981.
This terminal block type has not been used thus far for Mid-land, but will be used to replace Buchanan 0824.
f.
Cinch Jonas Series 140 This terminal bloc 1 la underrated for 460 V use. All such terminal blocks will be replaced by Marathon Series 300.
g.
Marathon 100 Series This terminal block is underrated for 460 V use.
All such terminal blocks will be replaced by Marathon Series 300.
h.
Marathon 300 Series This terminal block is correttly rated for 460 V use and has been qualified environmentally for in/outside containment use.
Limitorque will furnish complete documentation by December 31, 1981. No replacement is presently required.
k.
Beau 7600 This terminal block is correctly rated for 460 V use. This terminal block is not environmentally qualified for in-contain-ment use, and therefore will be replaced with a qualified terminal block for in-containment use if found to exist in containment. It is qualified by analysis for outside contain-ment use. Limitorque will furnish complete documentation by December 31, 1981.

General Electric EB-5 This terminal block is correctly rated for 460 V use and has
,f-been qualified environmentally for in/outside containment use.

Limitorque will furnish complete documentation by December 31, 1981. No replacement is presently required.

As agreed during the July 17, 1981, meeting, Limitorque has been provided with the specific environmental parameters for outside j
containment. This information has been furnished to Limitorque to j
evaluate the further suitability of the terminal blocks for outside containment use and will provide qualifying documentation. Limitorque also agreed to provide a replacement block for harsh environment areas outside containment, if required by Bechtel.
t Limitorque does not consider qualification testing within the scope of the specification for outside-containment service, but will supply 1
a report. The report will address acceptability of the terminal blocks for the intended service based on generally accepted industrial and/or Limitorque in-house standard design criteria. Marathon 300, Buchanan 0222, and GE EB-5 are the terminal strips for which Limitorque haa available documentation to demonstrate qualification for outside 4
containment.
Bechtel also provided a complete listing of Bechtel Q-listed purchase orders cross-referencing Limitorque orders with the vendors' list.
4.
Bechtel procurement will coordinate with the prime valve vendors and Limitorque for implementation of inspection and replacement of the underrated / unqualified terminal blocks as follows. Limitorque may use, at its own risk, a terminal block for replacement that it deems quali-fiable.
a.
On the basis of the random sampling inspection results, it is determined that underrated terminal blocks are present only on the operators on Purchase Order 7220-M-132(Q)-AC. Henry Pratt Company (valve vendor) will be responsible for inspection of all 71 operators on Purchase Order 7220-M-132 and replacement of underrated terminal blocks found during inspection with Marathon 30G Series terminal blocks.

b.
As mentioned above in Item 1, Buchanan 0824 were reported to exist on Purchase Order 7220-M-123A. All 12 operators f
supplied on this order will be inspected, and the unqualified i
terminal blocks will be replaced by Curtis Type L.
c.
Prime valve vendors will be responsible for inspection of operators e
located outside containment in harsh environments, and operators located in containment. There are one hundred valves located in these harsh environments. The unqualified terminal blocks found during this inspection will be replaced with qualified blocks.
The listing of these 100 valves it being provided to Limitorque.
Limitorque will provide a detailed report by December 31, 1981.
5.
The following outstanding concerns require resolution with y
a.
Limitorque is to establish a root cause for the presence of unqualified terminal blocks Buchanan 0524, Buchanan 0824, Marathon 300 Series, Beau 7600, Kulka 622, and General Electric EB-5 used on Limitorque operators inside and outside containment.
b.
Limitorque is to check its records to determine if Buchanan 0824 type terminal blocks have been supplied on any purchase order other than 7220-M-123A, and is to confirm the status.
The status of 17 Limitorque operators to be inspected (upon receipt 6.
at the jobsite) for underrated terminal blocks (Item 1, Interim Report 3, June 19, 1981, MCAR 46) is not yet available. The field will also be requested to inspect for unqualified terminal blocks.
j The status will be provided in next interim report.
The next report will be issued by January 29, 1982.
