Seymour, IN 47274 Gentlemen:

This refers to the routine safety inspection conducted by Ms. P. Whiston and Mr. J. R. Mullaue e of this office on October 20, 1981, of activities authorized by NRC Byproduct Material License No. 13-05605-01 and to the discussion of our findings with you and Messrs. Gary Meyer and John Sluss at the conclusion of the inspection.

The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.

During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.

A written response, submitted under oath or affirmation, is required.

We are concerned that the number and nature of the items of noncompliance appear indicative of a breakdown of your management controls over licensed activities. These concerns were discussed in a telephone conference on October 28, 1981, between you and other members of the hospital staff and myself and representatives of the Region III staff.

In addition to the inspection findings, we discussed the provisions of the NRC interim en-forcement policy.

In your response to this letter, please describe the management controls you will use to assure these items of noncompliance are corrected and do not recur. The enclosed Appendix B, Management Control, is provided for your information and guidance in responding to this concern.
