Appendix A NOTICE OF VIOLATION Columbia Hospital License.No. 48-02417-01 As a result of the inspection conducted on November 9,1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
1.
License Condition No. 16 states that the licensee shall possess and use licensed material in accordance with statements, representations and procedures listed in application dated.May 22, 1979, and referenced -
letters.
This application states that radiation level and contamination surveys will be performed in your Nuclear Medicine Department on a weekly basis.
Contrary to the above, no surveys were performed between July 24 and September 25, 1981.
In addition, only 15 surveys have been performed from January 1981 to the date of this inspection.
This is a Severity Level V violation (Supplement VII).
2.
License Condition No. 16 states that the licensee shall possess and use licensed material in accordance with statements, representations and procedures listed in application dated May 22, 1979 and referenced j
letters.
This application states that calibration procedures for your dose cali-bration will include quarterly tests for linearity.
Contrary to the above, no linearity checks were performed during the first and third quarters of 1979; nor during the second and third quarters of 1980.
This is a Severity Level V violation (Supplement VII).
3.
License Condition No. 16 states that the licensee shall possess and use licensed material in accordance with statements, representations and and referenced procedures listed in application dated May 22, 1979j letters.
This application states that your Radiation Safety Committee will meet at least quarterly.

Contrary to the above, your Radiation Safety Committee did not meet during the first and third quarters of 1979, the first quarter of 1980, nor the first and third quarters of 1981.
This is a Severity Level V violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, you are-~ required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) cor-rective action taken and'the results achieved; (2) corrective action ~to be taken to avoid further noncompliance; anit (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Consideration may'be given to extending your response time for good cause shown.
