NOTICE OF VIOLATION Long Island Lighting Company Docket No. 50-322 Shoreham Nuclear Power Station License No. CPPR-95 As a result oi'the inspection conducted on August 11-14, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion VI, states, in part, that; " Measures shall be estabi;shed to control the issuance of documents,..."
FSAR Section 17.2.6 states, in part, that: " Internal LILCo organizations.
.., are responsible for establishing and implementing a document control system..." which " provide measures to assure the prompt distribution of approved changes and revisions including control of obsolete or superseded documents to prevent their inadvertent use."
In addition, "The LILCo QA Department and the station OQA organization are responsible for assuring overall program adequacy and implementation.
The LILCo Operational Quality Assurance Manual Section 6.2 and 6.3 states, in part, that:
"The managers of organizations..., are responsible for imposing the applicable requirements of this Section on their internal operations...." and " Established procedures..
.shall assure the prompt distribution of all approved changes and revisions including provisions for retrieval or destruction of superseded documents to prevent their inadvertent use."
The LILCo Startup Manual Section 1.3 states, in part, that: "To maintsin control of the Startup Manual, the distribution of controlled copies must be definite."
Contrary to the above, on August 13, 1981, Startup Mt.4ual No. 43, located in the control room and used by persons in the control room, was not adequately controlled in that i+ was not updated to include:

In addition, the " Controlled" Manuals Distribution List posted in revision 12, dated February 18, 1981 incorrectly assigned Startup Manual Copy No. 43 to a different recipient.
This is a Severity Level V Violation.
(Supplement II of the Interim Enforcement Policy)
Pursuant to the provisions of 10 CFR 2.201, Long Island Lighting Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) i the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.
The responses directed by this Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
