APPENDIX A NOTICE OF VIOLATION Vermont Yankee Nuclear Power Corporation Docket No. 50-271 Vermont Yankee Nuclear Power Station License No. DPR-28 Vernon, Vemont As 'a result of the inspection conducted on August 11 - October 5,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),
the following violations were identified:
A.
Technical Specification 6.7.B.2 requires that when conditions occur that lead to operation in a degraded mode as pemitted by a limiting condition of operation, a written report describing the event shall be submitted to the NRC within 30 days of the occurrence of the event.
Contrary to the above, Standby Gas Treatment Train B was declared inoperable on March 6,1981, a condition conctituting operation in a degraded mode, and licensee event report 81-19 was not submitted to the NRC until August 20, 1981.
This is a Severity Level VI Violation (Supplement I.F).
B.
Technical Specification 6.5.A. requires that procedures be established and implemented to provide control of activities involving plant operation and surveillance testing, and, that these procedures be followed. Plant pro-cedures OP 2124 and 4117 were developed pursuant to the requirements of Technical Specification 6.5.A. to provide the control of activities involving y the Residual Heat Removal (RHR) and Standby Gas Treatment (SGT) systems, re-spectively. OP 2124, Appendix A, specifies that valve FCV-43 be closed when the RHR system is aligned for standby operation. OP 4117 specifies that valve 4B be closed following the 10 hour1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br /> surveillance test of SGT Train B.
Contrary to the above:
OP 2124 was not followed in that RHR System valve FCV-43 was found OPEN at 6:00 P.M. on September 22, 1981, with the RHR System aligned for standby operation.
OP 4117 was not followed in that SGT valve 4B was found OPEN at 3:45.
P.M. on August 17,1981, after surveillance testing had been completed and SGT Train B was aligned for standby operation.
These items collectively represent a Severity Level V Violation (Supplement I.E.).

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affimation. Where good cause is shown, consideration will be given to extend your response time.
The responses directed by this Notice are not subject to the clearance procedures of the Office of Management arid Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
