50-267 NOTICE OF VIOLATION Based on the results of an NRC inspection conducted during the period of September 1-30, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66745 (October 7, 1980), the following violations were identified:
1.
Technical Specification 7.4.a, " Procedures, Administrative Controls,"
states in part that, "... written procedures shall be established, implemented, and maintained...."
Procedure ASP-3, " Maintenance Quality Control Program," effective date August 18, 1981, Section 4.1, " Maintenance and Preventive Maintenance Procedures Control," states in part that,"...
Hold Point:
An inspection point in a procedure where the crafts-man has notified the Maintenance Quality Control Technician, who in turn must sign off the inspection before work shall be allowed to continue.... "
Contrary to the above, on September 15,1981, at 11:00 a.m. (MDT), the NRC inspector determined that five hold points for work previously per-formed had not been signed by the Maintenance Quality Control Technician.
This is a Severity Level V Violation (Supplement I.E.).

Technical Specification LC0 4.4.1, " Plant Protective System Instru-mentation," Table 4.4-1, Item 4,"PrimaryCoolantMoisgure, Loop Monitor," specifies an absolute low trip setting of 27 F which is equivalent to a setting on each of the instruments at 3.09.
Contrary to the above requirements, at 10:00 a.m. (MDT), on September 23, 1981, MM1117 was observed to be at a setting of 5.09 and MM1121 was observedgobesetatasettingof3.29.
A5.09settingisequigalent to a 92.5 F dewpoint, and a 3.29 setting is equivalent to a 29.3 F dewpoint.
This is a Severity Level IV Violation (Supplement I.D.2).

10 CFR Part 2.201(a), Part 2, Title 10 of the NRC's " Rules of Practice,"
states in part that "... the notice of violation will concisely state the alleged violation and will require that the licensee submit, within 20 days of the date of the notice or other specified time, a written explanation or statement in reply including:

2 Contrary to the above, tne licensee did not reply within the specified times of 25, 30 and 30 days, respectively, for Notices of Violation for Inspection and Enforcement Reports 81-11 dated June 25, 1981, 81-12 dated June 15, 1981, and al-13 dated July 23, 1981.
This is a Severity Level IV Violation (Supplement I. D.2).
Pursuant to the provisions of 10 CFR Part 2.201, Public Service Company of Colorado is hereby required to submit to this office within 30 days of the date of this !!atice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
Consideration may be given to extending your response time for good cause shown.
The responses directed by unis Notice a*e not subject to tie clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
