Appendix A NOTICE OF VIOLATION St. Mary Hospital License No. 12-11288-01 As a result of the inspection conducted on October 23, 1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

1.

License Condition No. 18 requires that all licensed material be possessed and used in accordance with statet ts, representations and procedures contained in applications dated Jul 11 and October 7, 1977; letter

-response dated June 26, 1978; letter requesting addition of Dr. Ravel dated June 26, 1978, and letters dated April 7, 1978, and May 6, 1980.

The letter dated April 7, 1978, states that the procedures described in the NRC Licensing Guide will be followed for area surveys. The NRC Licensing Guide states that weekly wipe tests shall be performed and recorded.

Contrary to this requirement, weekly wipe tests were not performed since the date of license issuance.

This is a Severity Level V violation (Supplement VII).

The letter dated April 7, 1978, states that a G-M survey shall be per-formed on all packages at three feet from the package surface and wipe j

tests shall be performed on the source container.

Contrary to these requirements, a G-M survey was not performed at three feet from the packa'e surface since the date of license issuance. Wipe g

tests on the source container were not performed since the date of license issuance.

This is a Severity Level V violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this offica within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:

(1) cor-rective action taken and the results achieved; (2) corrective action to be 81i1200501 811112 NMS LIC30 PDR 12-11288-01

a Appendix A taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or a f fi rmation. Consideration may be given to extending your response time for good cause shown.
