10CFR50.55(e) t 'j,;' g Inadequate Construction Controls Concerning Electrical Raceway Hanger Installation On Septesber 16, 1980, Illinois Power verbally notified Mr. H.M.

Wescott of a potentially reportable deficiency involving inadequate construction controls and electrical raceway hanger installation.

After conducting en investigation, the deficiency was considered reportable pursuant to the provisions of 10CFR50.55(e)(1) and reported in an interim report dated October 20, 1980.

Subsequent interim reports were submitted on December 19, 1980; February 25, 1981; May 6, 1981; and July 9, 1981.

This letter is a final report on the inadequate construction controls concerning electrical raceway hanger ins tallation, in accordance with the requirements of 10CFR50.55(e).

Statement of Reportable Deficiency A number of completed (constructed and inspected) and in-process (in various stages of construction or construcled and not inspected) electrical raceuay hangers were found to be in non-conformance with approved current revision design documents.

Investigation Summary The problems associated with electrical hanger installation constituted a deficiency in design / construction controls.

This deficiency resulted due to the complexity and format of the Sargent & Lundy information provided to the field and the interpretation of the engineering design in the field.

Difficulties were also encountered in documenting the physical "as built" condition of the electrical hanger.

Lastly, the process of effecting changes to constructed hangers was found to be inadequate.

' Corrective ~ Action Corrective action was directed toward upgrading hanger construction controls as necessary to (1) facilitate the

-accurate documentation of constructed hanger details, (2) identify and implement revisions to existing hangers, and (3) streamlining the design information provided to the field.

To effect the. corrective action, the entire process of hanger design and installation was reviewed.

Various Baldwin Associates' procedures and instructions were either written or revised to incorporate systematic changes and work controls for new and then-existing hanger installation work.

A computerized tracking system was developed to aid in identifying hangers affected by design revisions.

Lastly, training of supervision and craft personnel was conducted to ensure understanding of the revised hanger installation process.

Further details regarding the considerable efforts expended in developing and implementing revised design / construction controls are available for review at the Clinton site.

Safety Implication / Significance The electrical hangers support raceway systems which carry nuclear-safety-related cable and could possibly interact with other nuclear safety-related items.

Considering the complexity of hypothesizing various failure mechanisms, it is difficult to evaluate the hazard if any to public' health and safety if the deficiency had gone undetected.

This letter is hereby submitted as a final report in accordance with 10CFR50.55(e), and I trust that it is sufficient for your analysis and evaluation of the deficiency and corrective action.
