e APPENDIX A NOTICE OF 'IIOLATION Metropolitan Edison Company Docket No. 50-320 Three Mile Island, Unit 2 License No. DPR-73
- As a result of the inspection conducted on July 30 - September 28, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violation was identified.
(he Order for Modification of License, dated July 20, 1979, as amended by the Order dated February 11, 1 980, states in part:
...Pending further amendment of the Facility Operating License, the licensee shall maintain the facility in accordance with the requirements set forth in..." (proposed Technical Specifications -(TS), Appendix A to License No. DPR-73).
The propssed TS 6.8.1 requires in part that written procedures shall be implemented covering applicable procedures recommended in Appendix A 'of Regulatory Guide (RG) 1.33r Revision 2, February 1978, Appendix A of RG 1.33, paragraph 1.c requires administrative controls
- for equipment control (e.g. tagging).. Administrative Procedure (AP) 1002, Revision 18. February 11, 1981, Rules for the Protection of Employees Working on Electrical and Mechanical Equipment, paragraph D.3 requires in part that when a person reports " clear of work" under blue tags the CR0 (Contml Room Operator) shall note the status and posittor. of. the components that are blue-tagged on the clearance section of the tag application.
i-Contrary to the above, as of August 31, 1981 personnel reported " clear of work" under blue tagged components (safety related and important to safety systems) without associated valve / breaker positions being recorded along with documented status.
In other instances no status was documented.
