Appendix A NOTICE OF VIOLATION Nuclear Medical Imaging License No. 34-18843-01 As a result of the inspection conducted on October 13, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1930), the following violations were identified:
1.
License Condition No. 15 requires that licensed material be possessed and used in accordance with statements, represent:,tions, and procedures contained in application dated December 7, 1979, and letter dated January 14, 1980.
The above referenced av,'ication and letter require that the proce-dures /.escribed in Apr
< D, Section 2 of the NRC Regulatory Guide 10.8 be followed for o aration of the dose calibrator using certain specified sources. These procedures require barium-133 be used as one of the reference sources for the accuracy check and that quarterly linearity checks be performed.
Contrary to these requirements, linearity checks have not been per-formed quarterly and accuracy checks have not been performed using appropriate sources. Specifically, the last linearity check was done in February 1980 and accuracy checks have been performed without the use of a barium-133 standard.
This is a Severity Level V violation (Supplement VII).

License Condition No. 15 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated December 7, 1979.
The above referen;4d application, Item 14 requires that the procedures described in Appendix F of the NRC Regulatory Guide 10.8 be followed for safely opening packages containing radioactive materials. These procedures require direct surveys and wipe tests of incoming shipments of radioactive material.
Contary to the above, no surveys or wipe tests have been performed on incoming packages containing radioactive materials since the inception of this requirement.
This is a Severity Level V violation (Supplement VII).

Appendix A Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) cor-rective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
Consideration may be given to extending your response time for good cause shown.
