Dear Mr. Haynes:

Reference (1) informed your office of an unsatisfactory surveillance test on one of three pressurizer code safety valves. Preliminary information at this time indicated the possibility that the valve may have been damaged during the surveillance test sequence. The manufacturers on site representative has confirmed that the valve bellows assembly was damaged during the surveillance test sequence.

We therefore conclude that the valve was operable during power operation preceding the refueling shutdown and no further report is required.
