SCOPE: This inspection was made as a result of the observation by Virginia Electric Power Company (VEPCO) of oil leakage from a North Anna Unt 3 Auxiliary Feedwater Pump Motor during storage.
Subsequent examination of the motor, which was manufactured by the Westinghouse Large Motor Division (WLMD),
showed that the leakage had occurred because of the presence of casting defects in the lower reservoir of the shaft drive end bearing. The inspection scope was limited to follow up cn the identified deficiency.

Appendix B to 10 CFR Part 50 was not imposed by the purchaser (Bingham Willamette Co.) in the procurement requirements for the Auxiliary Feedwater Pump Motor which subseque tly exhibited oil leak 3ge during site storage.
In response to questions concerning the differences in quality assurance requirements for commercial and safety grade motors, WLMD identified the differences as consisting essentially of the scope of activities for which documentation is prepared.
Examples noted of additional requirements imposed for safety grade motors were as follows: (a) consideration of 10 CFR Part 21 applicability in procure-ment; (b) documented inspections; and (c) use of separate manufacturing shop orders.

The NRC inspectors were unable to verify that original inspection had been performed of the end bearing casting which was subsequently established to contain casting defects, in that no receiving inspection records are maintained for commercial motor items. The WLMD QA Manager indicated that an inspection had been performed of the warehouse stock of end bearing castings as a result of the oil leakage problem. No doc 2mentary evidence was available, however, which would allow confirma-tion of this information.

During the inspection:
a.
The NRC inspectors visually inspected Neenah Foundry Company 709J188C13 and 2235C07C04 end bearing castings in the warehouse and on the factory floor. No defects similar to that which caused the oil leak in the 709J188Cl3 North Anna motor casting; i.e., unfused " pins" or " chaplets" were observed.
A liquid penetrant examination was performed by WLMD of a o.
709J188C13 casting at the request of the NRC inspectors.
No defects were found of a type which could have resulted in oil leakage.

The NRC inspectors were shown documentary evidence that the motor with the oil leak and three other WLMD motors, which had been supplied with it by the Bingham Willamette Company, had been returned on April 15, 1981, to a Westinghouse Service Repair Shop (WSRS) in Richmond, Virginia, for examination and repair.
WSRS dismantled all four motors and VEPC0 examined the end bearing castings.
Remnant chaplets were observed in all eight end bearing castings in the motors and evidence of oil leakage was visible in three. WSRS ground down all of the chaplets and seal welded the exterior of the castings by the gas tungsten arc welding process.
Sealing of the defects was completed using Belzona and a dye penetrant.
Examination was then performed to verify freedom from leaks. Repair operations were then completed by priming the reservoir interior surfaces and epoxy painting.

WLMD management informed the NRC inspectors that there was no previous history of this type defect and that the problem was considered an isolated case.
No evidence was made available to the NRC inspectors that would indicate any actions had been taken to preclude recurrence of similar problems.
