This refers to Certificate of Compliance No. 9074 for the Model No. AP-100 packaging which expires on March 31,1982.

Because the certificate reTerences consist of an application and many

- supplements, we riquest that you submit a consolidated application, with drawings, incorporating all previon submittals. The consolidated application need only reference your overall QA program but should include all quality control requirements and operating procedures for the Model No. AP-100 packaging.

In order to provide for continued use of this packaging, the consolidated-application (8 copies) shouisi be submitted at least thirty (J0) days prior to the expiration date of the certificate.

In addition, the renewal fee prescribed in 10 CFR 5170.31'should be included with your request.
