4 Appendix A NOTICE OF VIOLATION Dairyland Power Cooperative Docket No. 50-409 As a result of the inspection conducted on September 8-11, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following v!91ation was identified:
Technical Specifications 6.8.1 and 6.11 require adherence to radiation protection procedures.
LACBWR Health and Safety Procedure 02.6, Section 7.11 requires areas with radiation fields which exceed the surrounding general area radiation fields by a factor of three in dose rate to be posted " Caution Radiation Hotspot."
Contrary to this requirement, an area of the fuel element storage well piping on the mezzanine level of the Containment Building read in excess of 600 milliroentgens per hour (mR/hr) which exceeded the radiation field of the surrounding general area by a factor of three and was not posted with a " Caution Radiation Hotspot" sign.
This is a Severity Level V violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) cor-rective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this responac shall be submitte-1 under oath or affirmation.
Consideration may be given to extending your response time for good cause shown.
