We would appreciate written confirmation of the receipt of our application for renewal of license #SNM-1513, dated April 10, 1981.
Since this application was submitted more than 30 days before the license expiration date (July 31, 1981), we assume that the license is still valid as per Section 70.33(b).
Also, please direct future correspondence to the RSO at the address below. A notice of license expfration (copy attached), O ted April 3, 1981, was received by me on August 27, 1981. Presumably, part of the delay was due to NRC's use of the street address show1 on the license. Unfortunately, "1669 Lake Avenue" is the street address used for Kodak Park, a facility incorporating about 33,000 people in several hundred buildiags extending over several miles.
It is imperative that all correspondence be specifically addressed to the Corporate Radiation Safety Officer using the approved mailing address shown below.
