AP.26, Abnormal Tag Procedure, Step 3.2 (July 8, 1981), in that nine abnormal tags over a year I

old did not have NCR's written on them.

c.

A.51, Auxiliary Feedwater System, Step 4.2.4 (July 9,1981) in that the locking devices for four valves were not functional.

d.

SP 214.03, Locked Valve List, Step 4.2 (July 28, 1981) in that the locking devices for two valves were not functional.

This is a Severity Level 5 violation.

District Reply item a The District acknowledges that the seven abnormal tags were io t logged in the l&C shop abnormal tag log; however they were logged in the Computer Shop area. The District recognizes that the log in the Computer Shop area sas r.ot desc'ibed in AP.26, therefore AP.26 was revised.

The current revision (Revision 4, dated July 24, 1981) includes the Computer Shop log as a location for logging abnormal tags.

Item b The Dist rict acknowledges that the nine tags rentioned did not have NCR's wri t ten on them.

To prevent this f rom recurring, tiie abnormal tag sogs w l l be reviewed monthly to identify tags approac..ing the i

twe.ve-month limit. Tags that are eleven months old will be idertified for NCR's.

Items c and d Tne District acknowledges that the valves mentioned w. re not properly secured. This was due to the use of smooth wirc in some cases, which allowed the seals to slip, and others were no, properly crimped.

All did have the securing wire, however. The Dittrict recognizes that the locking devices must be secure, therefore the following action will he taken during the upcoming startup:

a.

Smooth wire will be changed to ttainless steel braided wire on any valves that are noted to have slippage on the locking devices.

b.

Smooth wire will be replaced with braided as valves are manipulated and tags are replaced.

Special Order 50 8-81 has been issued instructing Shift Supervisor's crews as to the importance of proper instal -

lation of the lead seal (crimping) and the locked tags as per SP 214.03, and explaining the use -f braided wire.
