' NUCLEAR INDUSTRY ACTIVITY: TECH-SIL, Inc., (TSI) is currently providing materials, equipment, engineering and design technology, and installation of fire and radiation seals for mechanical and electrical penetrations, either in total or in part, at fcur US nuclear power generating stations.

Past work has been performed at seven US nuclear power generating stations. Approximately 98% of their work is devoted to the commercial nudlear industry.

SCOPE: This inspection was made as a result of penetration seal materials being provided to the LaSalle County Station and the William H. Zimmer Nuclear Power Station, with formulations different from that tested for fire resistance, and concerns expressed over the adequacy of qualification testing of flexible penetration boots that have been furnished to the Grand Gulf Nuclear Station, Unit 1.

Additional areas selected for inspection included: material identi-fication and control; procurement control; equipment calibration; Snd control of nonconformances and corrective actions.

VIOLATIONS:

1.

Contrary to paragraph 21.6 of 10 CFR Part 21, posting of 10 CFR Part 21 and Section 206 of the Energy Reorganization Act of 1974 or an appro-priate notice had not been accomplished.

2.

Contrary to paragraph 21.21(a) of 10 CFR Part 21, procedures had not been adopted to provide for: (a) evaluating deviations or informing the licensee or purchaser; and (b) assuring that a director or responsible officer was informed if the supplied basic component (1) failed to comply, or (2) contained a defect.

B.

NONCONFORMANCES:

1.

Contrary to Criterion V of Appendix B to 10 CFR Part 50, QA Manual Section XVII, and Procedure QC-201.3, certain records did not furnish evidence of activities affecting quality, as shown by the following:

The Systems and Test Log for the Hi Density (leaded) Elastomet s using formula number TS-MS-0060E showed that 1631 lbs. of 170 Elastomer "A",

lot number EPO 41861 were used on July 27 and 28, 1981, and 1048.5 lbs.

of 170 Elastcmer "B", lot number EP0 41880 were used en July 28 and 29, 1981.

However, a review of Receiving Reports showed that TECH-SIL, Inc., had received just 1500 lbs. of 170 Elastomer "A", lot number EP0 41861, and 1000 lbs. of 170 Elastomer "B", lot number EPO 44880, both of which were on Receiving Report No. 364,~ dated July 1, 1981.

2.

Contrary to Criterion V of Appendix B to 10 CFR Part 50 and paragraph 3.0 of QA procedure 108 in the QA manual, the calibration of scales used in the formula mixing process could not be certified to a standard traceable to the National Bureau of Standards.

3.

Contrary to Criterion V of Appendix B to 10 CFR Part 50 and Section XV of the QA manual, the actual disposition and subsequent verification of rejected material could not be determined, in that these activities are not documented.

OTHER FINDINGS OR COMMENTS:

1.

Penetration Seal Materials With Formulations Different From Those Tested' For Fire Resistance The penetrations at LaSalle County Station and William H. Zimmer Nuclear Power Station, in which this seal material was used, have been identified, with subsequent removal and replacement of the seal material.

The balance of the seal material is accounted for and is identified and segregated at the Elk Grove Village facility.

Qualification Testing of Flexible Penetration Boots - TSI had qualified the penetration boots in regard to fire test requirements. However, due to concerns over the adequacy of the tests, TSI had additional tests performed on the High Density Silicone Gel / Flexible Boot combination.

The boot and gel were irradiated for a 40-year equivalency, then fire tested and cycle tested. The fire and cycle tests were witnessed and approved by Portland Cement Association-Construction Technology Laboratories Division, with the test reports being updated April 29, 1981. The irradiatian tests were perforned by the University of Michigan Phoenix Memorial Laboratory.

Material Identification and Control - This area was evaluated by review of receiving reports, material test reports, mixing systems and test logs, and observation of stored materials which were identified by lot / batch numbers. Nonconformance A was identified.

4.

Procurement Control - This area was evaluated by inspecting purchase orders, certificates of analysis, vendor assessment sheets, and material certifications. No deficiencies were observed, however, the chemical compositions of the purchased elastomers were not shewn on the accompanying certificatcs of analysis.

5.

Equipment Calibration - This area was evaluated by inspecting scale logs and calibration records. Nonconformance B was identified.

6.

Control of Nonconformances and Corrective Actions - This area was evaluated by inspecting Nonconforming Condition Reports and Minor Variation Reports from several site cperations and the formula mixing facility.

Nonconformance C was identified.
