Appendix A NOTICE OF VIOLATION General Instrument Corporation License No. 12-19460-01 Lamp Division License No. 12-19460-02E As a result of the inspection conducted on September 22, 1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:

License No. 12-19460-01 1.

License Condition No. 13 requires that all licensed material be possessed and used in accordance with statements, representations and procedures contained in application dated May 23, 1980.

The application referenced above states that the Texas Nuclear G-M survey meter shall be used as the radiation detection instrument at the Chicago Facility.

Contrary to this requirement, on September 22, 1981, you failed to have a G-M survey meter at your Chicago Facility.

This is a Severity Level V violation (Supplement VII).

License No. 12-19460-02E 2.

License No. 12-19460-02E requires that the packing slips accompanying lamp and tube shipments and the outer container of the packages containing the lamps and tubes shall be marked with the identification of the manufacturer and the byproduct material contained in the lamps and tubes.

Contrary to this requirement, the packing slips accompanying lamps and tubes were not marked with the byproduct material contained in the lamps and tubes.

This is a Severity Level VI violation (Supplement VII).

License No. 12-19460-02E requires that you file an annual report with the Chief, Materials Licensing Branch, Division of Materials and Fuel Cycle racility Licensing, U.S. Nuclear Regulatory Commission, Washington, D.C.

20555, which states the number of units of each type of product distrib-uted and the quantity of krypton-85, contained in the products. Each report shall cover the year ending June 30 and shall be filed within 30 days thereafter.

O Appendix A Contrary to this requirement, you failed to file the required report for the year ending June 1981.

This is a Severity Level VI violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation la aply, including for each item of noncompliance:

(1) correc-tive action taken and the results achicved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.

Con-sideration may be given to extending your response time for good cause shown.
