-Appendix A NOTICE OF VIOLATION Luther Hosptial License No. 48-02122-04 As a result of the inspection conducted on September 28,-1981,.and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
1.
The order modifying teletherapy licenses dated May 7, 1980,. states that whenever your area monitor is not operational, any person entering the teletherapy room following an irradiation shall enter with an operable and calibrated radiation survey meter to determine the beam condition.
Contrary to the above requirement, it was determined through state-ments by licensee representatives that this requirement is not being met.
Specifically, a licensee representative stated the Primalert l'" Model 05-433 was sent back to the manufacturer for repairs'on 0
September 8, 1981.
In the absence of the area monitor, personnel entering the teletherapy room following irradiation have not been determining the condition of the beam with the required survey instrumentation.
This is a Severity Level V violation (Supplement VII).

10 CFR 35.25(a) and (c) requires the licensee-to maintain for.
inspection by the Commission records of calibration-of the instru--
ments used to make full calibrations and records of the licensee's evaluation of the qualified experts training and experience.
Contrary to the r.bove requirement, it was determined through state-ments by licensee representatives that this condition is not being met.
Specifically, on the day of this inspection, the above records were not maintained for inspection.
This is a Severity Level VI violation (Supplement VII).
