Appendix A NOTICE OF VIOLATION Dickenson County Memorial Hospital License No. 21-18889-01 As a result of the inspection conductel on September 30, 1981, and in ac-cordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),

the following violations were identified:

1.

License Condition No.15 requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in certain refc enced applications and letters.

The referenced application dated February 2, 1980, states in Item 15 that disposet'.c gloves will be worn at all times by all personnel when working with radioactive materials.

Cont _.ry tc ee above, on September 30, 1981, the inspector observed a technologioc, who was not wearing gloves, injecting radiophar-maceuticals.

This is a Severity Level V violation (Supplement VII).

License Condition No. 15 requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in certain referenced applications and letters.

The referenced application dated February 2, 1981, states in Item 17 that arca survey procedures described in the Guide for Preparation of Applications for Medical Programs, dated November 1977, Appendix I, will be followed. Appendix I requires all elution, preparation and injection areas tc be surveyed daily with a G-M survey meter.

Contrary to the above, areas used daily for injectin8 radiophar-maceuticals, such as the camera room and occasionally the west wing conference room, have not been surveyed daily since the date of license issuance ou April 21, 1980.

This is a Severity Level V violation (Supplement VII).

3.

Licens2 Condition No. 14.c. states the licensee shall establish written procedures for personnel performing tests to detect and quantify molybdenum-99 contamination in technetium-99m eluates.
