This refers to the routine safety inspection conducted by Mr. J. R. Mullauer of this office on August 31, and September 1, 1981, of activities authorized by NRC Byp oduct Material Licenses No. 13-08615-04 and No. 13-08615-05 and to the discussion of our findings with Norman R. Greene, David A. McEwen, M.D.

and other members of your staff, and the subsequent phone conversation with Mr. John Stevens on September 9,1981.

The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.

In addition to the above areas, the inspector examined corrective actions described in your letter daced February 5,1979, regarding two apparent items of noncompliance found during our December 7,1978, inspection of License No. 13-08615-04. Your letter described the action which would be taken to avoid further noncompliance; however, during the course of this inspection, our inspector identified one repeat item of noncompliance as specified in the attached Appendix A, Item No. 1.

During this inspection, no items of noncompliance were identified for License No. 13-08615-05. However, for License No. 13-08615-04 certain of your activities appeared to be in noncompliance with NRC requirements, as described in enclosed Appendix A.

A written response, submitted under oath or affirmation, is required.

This inspection also included a measurement of the output of your tele-therapy equipment. Our measurement was made using a Victoreen Model 570 Condenser R-Meter with Model 621 chamber, calibrated by the National Burean p

OCT I 51981 Saint Elizabeth Hospital Medical Center of Standards Laboratory. The source-to-chamber distance was 00.5 centimeters and the field size used was ten by ten centimeters. After applying standard correction factors fo* temperature, pressure, and timer error, our measurement of the output si your teletherapy equipment was 141.1 roentgens per minute.

The most recet-fell calibration of your teletherapy equipment, conducted on July 7,1981, re3 7 :ted a measured output for the same exposure parameters of 140.2 roentgens per minute. Applying a correction factor to account for 56 days decay, the value calculated from the July 7, 1981, calibration becomes 137.4 roentgens per minute.

Based on the close agreement in raeasured output values, we feel the actual output of your teletherapy equipment to be within established guidelines of 15 percent.
