Appendix A NOTICE OF VIOLATION Menominee County - Lloyd llospital License No. 21-18523-01 As a result of the inspection conducted on September 29, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
1.
License Condition No. 14 requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in the application dated April 10, 1978.
The April 10, 1978, application states in Item 10 that a 250 micro-curie bar ium-133 calibration source will be used for accuracy tests of the dose calibrator.
Contrary to the above, accuracy tests performed since the date of license issuance have not included the use of barium-133.
This is a Severity Level V violation (SL:plement VII).

License Condition No. 14 requires that licensed material be possessed and used in accordance with the statements, representations, and proccJures contained in the application dated April 10, 1978.
Item 10 of this application states dose calibrator calibration frequencies defined in the Guide for Preparation of Applications for Medical Progrm s, dated November 1977, Appendix D, Section 2 will be followed. Appendix D, Section 2 requires linearity tests to be performed quarterly.
Contrary to the above, cone calibrator linearity tests were not performed during the fi ;st two quarters of 1980, nor during the first quarter of 1981.
This is a Severity Level V violation (Supplement VII).
3.
License Condition No. 14 requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in the application dated April 10, 1978. This application states in Item 16. F, that survey procedures in Appendix I of the Guide for Applications for Medical Programs, dated November 1977, will be followed. Appendix A G-M radiation area surveys will be performed daily in the hot a.
laboratory, dose preparation and administration areas, and weekly in all other lab areas.
In addition, wipe tests for removable contamination will be performed weekly in all lab areas, and b.
Records will be maintained of all surveys.
Contrary to the above, (a) weekly G-M and wipe surveys have not been performed since August 7,1981, and (b) no records have been maintained of daily hot lab surveys since license issuance, on April 26, 1978.
This is a Severity Level V violation (Supplement VII).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be sub-mitted under oath or r.firmation.
Consideration may be given to extend-ing your response time for good cause shesn.
