Appendix A NOTICE OF VIOLATION Menorah Medical Center License No. 24-01239-01 License No. 24-01239-02 As a result of the Jnspection conducted on September 16 and 17, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
the following violations were identified:
License No. 24-01239-01 1.
License Condition No. 15 stetes that the licensee shall possess and use licensed material in accordance with statenents, representations and procedures listed in applications dated September 27, 1977 and January 25, 1978, letter dated December 16, 1977. The September 27, 1977, application states that your dose calibrator will be calibrated in accordance with procedures described in the licensing Guide for Medical Programs, dated February 1976, which requires daily checks for constancy.
Contrary to the above, daily constancy checks were not performed between January 14 and 21; April 14 and 21; July 1 and 9; July 28 and August 4, 1980; and on several other occassions.
This is a Severity Level V violation (Supplement VII).
License No. 24-01239-02 2.
License Condition No. 14.A. states that teletherapy sources shall be tested for leakage at intervals not to exceed six months and that records of test results shall be maintained for inspection by the Commissi.on.
Contrary to the above, only one leak test record, dated March 10, 1981, was available for inspection.
This is a Severity Level VI violation (Supplement VII).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:

Appendix A achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
