Dear Sir:

his sunnittal is in response to NRC letter from Mr. V. Stello, Jr., to Mr. J. P. Bayne, Dated July 31, 1981. %e Authority concurs with the findings in the Notice of Violation and proposed Unposition of civil penalty appended to the above mentioned letter. Accord-in3 y, enclosed herewith is a check in the amount of $40,000.

1 Se reasons for these incidents were extensively discussed with the Resident Inspector and in a meeting held at the Region I offices on April 29, 1981. Refer to Inspection Report 50-333/81-07 dated July 31, 1981 and NBC Meeting Notice 50-333/81-11, dated June 3, 1981. Upon identification of this incident, a safety evaluation was performed which irdicated that no safety hazard existed. Changes to the 'Ibchnical Specifications were proposed to reflect the new setpoint configuration and approved by the N1C for a limir.ed time period. %e SRV in question has been returned to its original setpoint of 1140 psig as required by the 'Ibchnical Specifications.

1 In order to prevent these types of incidents from recurring, the Authority has implemented the following actions:

I.

a)

Procedures for the control of maintename and modifications WACP 10.1.1, have been revised to provide direction to plant personnel to insure that plant changes /madifications are reviewed, approved and documented and the resulting inpacts considered, mis revision has been completed.

. b)

A review of safety related work performed over the last 30 months by Quality Assurance to identify similar situations has been completed. No additional incidents were found.

c)

A review of the technical specification amendments has been coMucted by the Shift Technical Advisors to assure that all comitments wOhin the safety evaluations have been identified and incorporated as required. This review has been completed. As a rerult of this review, certain minor corrections to the Technical Specifications may be proposed and subnitted to the NRC for approval.

d)

An amendment to tne Technical Specifications was sutxnitted on April 28, 1981 to indicate in Paragraph 1.2 and 2.2 " Bases", that the SRV setpoint configuration in Paragraph 2.2.B is required to ensure Mark I containment structural integrity. his amendment to the Technical Specifications was approved as amendment 58 on August 12, 1981.

e) he Authority is in the process of establishing an independent onsite review committee to be staffed by one supervisor and three engineers reporting to the Headquarters Office. W is committee is scheduled to be in place prior to coamencing Cycle 5.

II.

a)

- Quality Assurarx:e procedure QAP 10.1 " Inspection of Quality aelated Activities", Paragraph 6.9 has been revised to include the following statement:

If the deficiency can ce resolved between the supervisor resprasible for tne work and the Quality Control Supervisor, resolution shall be documented and referenccd on the appropriate QCIR.

b) he Quality Assurance Superintendent has issuM a mmorandum to Qtv/QC personnel reemphasizing that procedures must be followed, and a Deficiency and Corrective Action Report (DCAR) shall be initiated to document any observed non conformanrc. This memoraMum furtner stated that this action should be followed regardless of any verbal' approval to waive such requirements, by the QA Superintendent or his supervisors.

Tne Authority considers that the above measures, will reduce the likelihood of an incident of this type from recurring.

Nevertheless, the Authority will continue to nonitor plant activities to ensure the adequacy of the corrective actions and to identify additional measures if needed.
