The certificate will expire on December 31, 1981. Because the certificate references consist of an application and many supplements, we request that you submit a consolidated application, with drawings, incorporating all previous submittals. The consolidated application need not include your QA program but should continue to include quality control requirements and operating procedures.
The consolidated application (8 copies) should be submitted at least thirty (30) days prior to the expiration date of the certificate.
In addition, the renewal fee prescribed in 10 CFR 5170.31 sholld be included with your request.
